TCAR Revascularization

Vascular surgeons at Jefferson Surgical Clinic have an unparalleled record in treating carotid artery disease using surgery or endovascular stenting.

Jefferson Surgical Clinic’s physicians now perform a new procedure called  TCAR, which combines surgical and endovascular techniques and allows for the treatment of patients deemed high risk for traditional carotid surgery.

The minimally invasive procedure involves placing a small incision at the base of the neck to allow direct access to the carotid artery. This allows for the safer delivery of the stent to the diseased artery. Traditional stenting involves the delivery of the stent from the groin, which can increase complication.

The TCAR procedure places a soft, flexible sheath directly into the artery and connects to the ENROUTE® Transcarotid Neuroprotection and Stent System, which reverses the flow of blood away from the brain – protecting it from pieces of plaque that may come loose during the procedure. This blood is filtered and returned through a second sheath placed in the femoral vein in the patient’s thigh.

While the blood flow is reversed with the Neuroprotection system, balloon angioplasty and stenting can be performed. When the stent is placed and stabilizes the plaque in the carotid artery, flow reversal is turned off to allow blood to flow to the brain as it normally does.

TCAR is a safer way to use stents to treat carotid artery disease. While it is a revolutionary procedure, TCAR at present is not intended to replace surgery. Rather, it’s meant to be a safer alternative to stent placement in patients deemed high risk for standard surgical repair.

Vascular surgeons at Jefferson Surgical Clinic use all modalities of therapy to ensure the safest and best outcomes.
